IHSAA SPORTS PHYSICAL AND ATHLETIC REGISTRATION TIME!

2024-25 Sports Physicals will be available for incoming 7-12th graders after school on Thursday, April 11th, from 3:30-6:30pm.  Time slots are every 15 minutes.

To schedule an appointment for a 2024-25 IHSAA Physical, contact Mrs. Hall in the athletic office at shall@lcss.org or 765-607-4918.  Cost for each physical is $25.00 cash or check.  Once scheduled, students or parents can pick up their physical form from the athletic office prior to the event.

Additionally, student-athletes must complete an IHSAA physical after APRIL 1st and register online for Athletic Participation via Final Forms at centralcatholic-in.finalforms.com prior to participating in conditioning, practice, and/or any interscholastic sport for the 2024-2025 school year.  All electronic forms MUST be signed by parent and student to be fully eligible.

Remember, FinalForms saves data from season-to-season and year-to-year for a quick registration experience!  Online registration is mandatory for all student-athletes next school year and beyond.
