If you’re new to the Lafayette Catholic School System, you MUST fill out a FERPA form. Please fill-out the form, and then return it via email to Natalie Stevens, Health and Safety Coordinator, at nstevens@lcss.org.

If you prefer to print it and fill-it out, you can either mail it to Natalie Stevens at 2410 s 9th Street Lafayette, IN 47909 or drop it off Monday-Friday from 9am-3pm at the Emmaus House Office on the south end of the parking lot at CC’s campus. LCSS FERPA Form
